I, the undersigned, give permission to Hauseit LLC to interview, film, photograph, tape or otherwise make a video or image reproduction of me and/or record my voice and use such photograph(s)/video(s) and any derivative works of these images and recordings in all forms of media, now known or later developed, for any and all promotional purposes.

I give permission to Hauseit LLC to use my first name and last initial and to use quotes from the interview(s) (or excerpts of such quotes), the film, photograph(s), tape(s) or reproduction(s) of me, and/or recording of my voice, in part or in whole, in all forms of media, for any and all promotional purposes. I understand that any video and/or images may appear with or without my first name and last initial.

I understand and agree that I will not receive any payment for my time or expenses or any royalty for the publication of the photograph(s)/video(s) or the use of my name and I hereby release Hauseit LLC and/or any parties designated by Hauseit LLC from any such claims.

I acknowledge that Hauseit LLC owns all rights to the images, recordings and any derivative work(s).

I hereby waive any right to inspect or approve the finished video recording(s), audio recording(s), film(s), soundtrack(s), photograph(s), printed matter or derivate work(s) that may be used in conjunction therewith or to the eventual use that it might be applied.

Since anyone can reproduce content online or from printed materials, I agree that Hauseit LLC is not responsible for unauthorized use by unaffiliated third parties of the images, recordings and any derivative work(s) produced.

I hereby release, defend, indemnify and hold harmless Hauseit LLC from and against any claims, damages or liability arising from or related to the use of the images, recordings or materials, and derivative works including but not limited to claims of defamation, invasion of privacy, or rights of publicity or copyright infringement, or any misuse, distortion, blurring, alteration or optical illusion that may occur or be produced in taking, processing, editing and producing content for publication or distribution.

This consent, waiver, indemnity and release is binding on me, my heirs, executors, administrators and assigns.

I certify that I have read and fully understand this consent and release, and that all questions pertaining to this consent have been answered to my satisfaction.
